I've worked at my current job for about two and a half years now. I have always requested days off as soon as I knew I needed the day(s). My supervisor, J, would always wait until days before the requested day(s) because she wanted to make sure I didn't change my mind or something happened and I couldn't take it. She told me to start making my request(s) the week of the requested day(s). So that is what I've been doing for about a year now.
Well, J left back in April(?) and E got promoted. Over the four months since E became my supervisor I've only requested two days: July 7 and August 14. July 7 I requested a week or so before because it was a Monday and August 14 I requested around two weeks before because I was taking the day to move my daughter into her dorm.
Months ago I knew I was going to want September 19 off (Maze Runner opens that day) but I knew better than to request it that far in advance. I didn't request it until today for reasons I will now explain:
Back in May I somehow got a whole month behind, as in I didn't do any paperwork for the entire month of May (absolutely no clue how that happened). I got caught up, mostly, in June. Despite having a four day weekend at the beginning of July, I was able to stay caught up until the end of August.
E got married and took the last week of August off for her honeymoon. This wouldn't be an issue except that no one updated the database we need to verify deposit slips that week and then we had Labor Day off.
This put me so far behind that I didn't finish August paperwork until Wednesday of last week and last week's paperwork late last Friday. I hate being behind at work so this means I tend to go to work when I shouldn't and don't take days off when I really need the mental health day.
I needed to make sure I would finish this week's work by end of day tomorrow which is why I waited until today to request it.
E usually approves requests right away but she didn't do that this time. I didn't think anything of it because she's been busy. But then a little after 12 I got this email:

I really wanted to ask why we were not informed previously about this rule change (like I said we have never had to do this before) and I've requested TWO days since she became my supervisor without requesting permission to request the day off (and how stupid is it that we have to get permission to ask for a fucking day off?)
I waited several minutes for her to approve my request but it became obvious she wasn't going to so I sent this:
I will not ever tell her why I need the day, although she does know why this time because we've discussed the movie (we've both read the series) and I wish I hadn't told her about when I'll be caught up. I know for next time.
Also, the only time I've requested for a day off during a blackout date is July 7 and I told her if I could have rescheduled the reason for the request, I would have (and no, I have never told her the reason because it's none of her business).

Did you email me regarding your PTO request for Friday? I do not have any record that you’re needing off.
My reply:
I was unaware that I'm supposed to email when I request it. I thought it sent you a notice when I submit the request.
Her:
I need to approve you being gone prior to your requesting the PTO. This is in case there is something that has to be completed prior to you leaving or if you are wanting to be off during a black out date. Verbal conversations are not sufficient because they are usually in passing and are easily forgotten – an email will guarantee that I have record of your request. The typical procedure is to request your PTO two weeks prior to the date you wish to take off – Lori’s doesn’t require this, but it would be appreciated.
Me:
I didn't know if I would be able to take the day two weeks ago.
Her:
I understand and said that I do not require you to give me two weeks, just that you submit your requests in an email to me prior to submitting your PTO request in TimeSaver. I will then respond and let you know to submit the TimeSaver request. I apologize that this was never reviewed; it hadn’t been a necessity.
